Christian Ed Program Registration Form

This registration form is for participation in Christian Education programs at St. John's.  We are working to reduce the number of forms that must be completed each year.  This form will address most needs; occasionally a special event may require a special form.

Guidelines:

• This form is to be completed by a parent or guardian.
• Please include on a single form all children in your household who share common parental contact and insurance information.  Different nformation = separate form.
• Most selections on this form are multiple choice; please select all that apply.
• Required information is indicated by a red asterisk *
• Out of necessity, the form is long.  To avoid wasted time, before you start to complete it, please be ready with:
  • All relevant phone numbers and email addresses
  • Your Doctor's name and phone number.
  • Your Health Insurance Company name, Group # and Policy #.

IMPORTANT:  WHEN YOU HAVE SUCCESSFULLY COMPLETED AND SUBMITTED THE FORM, a "Thank You" message will be displayed.  If you do not see the "Thank You" please scroll backwards to find "required" fields you may have missed.
